Care Coordinator

Work: Full Time

Job Summary:

Coordinates team-based care to provide health services to individuals, through effective partnerships with patients, their caregivers/families, community resources, and their physician. Facilitates a “shared goal model” within and across settings to achieve coordinated high-quality care that is patient- and family-centered.

Previous experience in caring for chronic disease patients required.

• 3-5 years experience in clinical or community health settings preferred.

• Previous Care Coordination, Case Management or Home Health experience preferred. Clinical nursing required.

• Demonstrates evidence of essential leadership, communication, education, collaboration, and counseling skills.

• Proficient in communication technologies (email, cell phone, etc.).

• Effective organizational skills and demonstrates ability to maintain accurate notes and records.

• Previous experience with health IT systems and data reports preferred.

• Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations preferred.

• Ability to identify and implement appropriate patient communication strategies and overcome accessibility barriers, as required.

Medical Billing Specialist

Job Summary:

 

The Biller is responsible for the timely input of information within the accounting systems at BHC. This role is directly involved in the maintenance of trust accounting records, census information, medical billing and resident accounts receivable statements for the nursing facility, and is the primary individual responsible for the timely collection of accounts receivable from Medicare and Medicaid.

 

 Job Requirements:

 

  • Demands/Working conditions: Thorough knowledge of entire billing process to include CPT-4, ICD-9 and ICD-10 coding.
  •  Capable of communicating effectively with patients and the health care team, including the ability to explain billing procedures and insurance benefits to patients.
  • Good computer skills and a working knowledge of Excel
  • Knowledge of and/or experience with billing and collecting from Medicare, Medicaid and commercial insurance plans along with knowledge of and/or experience with procedural (CPT) and diagnostic (ICD-10) coding and explanation of benefits (EOB) analysis.
  • Knowledge of legislative and private sector third party regulations and guidelines.
  • Knowledge of insurance agency operating procedures and practices.
  • Skill in exercising initiative, judgment, discretion, decision making and organizational skills to achieve business unit objectives.

 

 

 

MINIMUM QUALIFICATIONS:

 

Experience: First level accounting, or basic bookkeeping course or 2-3 years prior long term care bookkeeping experience is strongly recommended.

 

Education: High School diploma or equivalent.

 

Required experience.

 

  • Medicaid Billing: 1 year
  • Medicare Billing: 1 year
  • Long Term Care: 1 year
  • Electronic Medical Records : 1 year

Job Summary:

 

The purpose of the physical therapist is to assess patient ability or disability, prevent deformity or disability, relieve pain, develop and improve body skills, restore function, reach maximum performance within the patient’s capabilities.

Job Summary:

 

We are looking for a candidate with experience as a generalist. ‘Performs routine and moderate complex testing and analysis according to established procedures’ in all aspects of laboratory operations.

 

Job Requirements:

Duties: Perform routine chemistry, hematology, coagulation, urinalysis, immunoassay, rapid and molecular testing, blood bank and microbiology and proficient phlebotomy skills. Administrative duties such as; answer phones, maintain record and logs, organizational skills, time management, evaluate new procedures or methods, knowledge of laboratory regulations and standards.

Demands/Working conditions: Prolonged standing and walking, occasional heavy lifting (reagent boxes), ability to multitask in a busy lab.  The lab operation is Monday through Friday for inpatient and outpatient services as well as ER call nights, weekends and holidays.
