Patient Care Coordinator-Here To Serve You

Broadwater Health Center (BHC), through grant funding assistance, has hired a Patient Care Coordinator. Her name is Margaret Ruckey and she has a Bachelor of Science in Nursing (BSN), is a Registered Nurse (RN) and a Certified Emer-gency Nurse (CEN). The main job of a patient care coordinator is to ensure all preventative and current care options are being provided for and utilized by our Medicare patients. Margaret also wants to ensure patients understand every aspect of their care and they receive the highest quality of care possible.

So what exactly will Margaret be doing? Her role is to help the people of the community live healthier lives for as long as possible and she will start this process by working with Medicare covered patients performing the Annual Wellness Visits that are one of the many covered Medicare services. She is also in the process of developing a Chronic Care Management program that will help the people in our co-munity who are dealing with multiple medical issues to have better control of their health. She will also work with patients when they are discharged from BHC to help them transition from the hospital back home.
